Step 1

To set up an initial consultation, call to speak to one of our experienced front office staff members at the location nearest you. Be sure to have your insurance card in hand before placing the call.

We strongly advise new patients who are billing through insurance to call their insurance carrier and clarify their benefits prior to starting treatment to avoid unexpected financial hardship.

We kindly ask for a 24-hour cancellation notice. If not received, we charge $50 for the missed appointment.


Step 2

Please complete all the forms below and bring them to your appointment along with patient(s) insurance card, photo ID of patient/parent/guardian, and a method for co-payment.

We accept all major credit/debit cards and cash payment.
